Where the real harm happens
Recognise the four failures that actually cost people.
Most writing about AI ethics is about distant risks. This lesson is about the four that are hurting real businesses and real people this year.
Confident invention presented as fact. A model produces a case reference, a price, a medical dose or a legal deadline that does not exist, in the same tone as everything true. Lawyers have been fined for filing invented citations. The harm is not that the tool was wrong. It is that nothing in the output looked wrong.
Decisions made about people by a system nobody can explain. Screening job applicants, scoring loan applications, ranking tenants. If you cannot explain to the person why they were refused, you should not be using it to refuse them. This is a rule you can follow before any regulation arrives.
Other people's data going somewhere it should not. A staff member pastes a client contract, a patient list or a salary sheet into a free tool. Whether that data is then used for training depends on the plan and the terms, and by the time anyone checks, it has gone.
Impersonation. A generated voice or face used to make somebody appear to say something. This is where AI crosses from careless into harmful, and it is the one that ends careers.
Notice what these share. None of them requires an evil actor. All four happen when an ordinary person moves quickly and nobody set a rule beforehand.
